New Patient Registration

We are currently accepting new medical patients; however our new patient slots are limited and are assigned on a first come first serve basis.  Once you are a new medical patient, you may be referred.  Please call our office at 770-419-3120 Ext. 112 or 113 to inquire about the pre-registration process.

Actualmente estamos registrando pacientes nuevos en la clínica médica, sin embargo nuestros espacios para pacientes nuevos son limitados.  Por favor contacte nuestra oficina al (770) 419-3120 a la extensión 101 para solicitar informes del proceso de pre-registración.

Here's what we need you to do in order to become a patient.  Esto es lo que necesitamos que haga para convertirse en paciente.

1. Complete the new patient registration form and upload the necessary documents.  Complete el nuevo formulario de registro de pacientes y cargue los documentos necesarios.

2. Good Samaritan will call you to make your first appointment.   Good Samaritan lo llamará para hacer su primera cita.
